Wrasse Dead

Found my wrasse on the bottom of the tank limp almost with no life. I caught him and put him in a container that floated in the DT to see if he would make it. An hour later he was dead. He has been really healthy and eating good. He did good with all my other fish. They may get tired and chase him off sometimes but he seemed to have a good relationship. I tried to look over him and his eyes looked good, no spots on his body and I saw nothing on his gills. The only thing I've done different over the past few days was add some Mg to my tank. My Mg went from 1200 to 1260. Also added Ca. went from 380 to 420. Yesterday I also added some carbon via a media bag that sits in my sump sock filter. And did my weekly cleaning and water change. I've also got some GFO in a media bag in the sump sitting in a sock filter. Just trying to figure out what happened or what to look for in my other fish. I've turned on my UV sterilizer also. All my other fish look fine. No spots, no cloudy eyes, No fin tear, no strange movement. Any ideas? All parameters are good....
 
None of that should have been adverse. At all.
One thing that is worth looking at in wrasse deaths, (also for firefish, dottybacks, gobies) is the possibility of an unaccounted for pistol shrimp. These fish burrow. So do pistol shrimps. Mostly there is one red spot to tell the tale, but a blow to the skull could kill without a mark.
