Yellow tang acting weird please help!

Hey Guys hes still eating fine and swims and plays with other inhabitant like normal but every night he starts to get all crazy and breathe heavily. Last night I did a 50 percent water change thinking it was ammonia int he tank, my water test came back with Zero, Nitrates were at 20 ppm but they have been that way for over 2 months now. I cant seem to get it lowered. any suggestions.
My tank does have a back mirror and others have said maybe he sees his reflection but he also is very white around the front of his face and his fins looked clamped like something attacked it but there is nothing but 2 clowns and a cleaner shrimp in there:headwallblue:

im using premade saltwater from LFS and when I have to top off I just use NYC tap water but I let it sit for a few hours to get room temperature.
 
How's your oxygen level? Do you have a strong skimmer? A fuge? It could be oxygen shortage, which ultimately could become fatal.
