Anemone Problem...need help

Hello all, after 6 months in with my 90 gallon I was confident enough that my tank was ready for anemones. I had little to no trouble to date and all my parameters have been stable. Calcium about 420, Alk 8.2. Mag 1300. I have a pair of true perculas in there, one of which has hosted with both my toadstool leather coral and my cauliflower colt.

I added a green BTA and a rose one about a week ago. I did not quarantine either one of them. Both specimens came from a reputable LFS and looked great for the first day or two. The green one was a bit larger and I removed it from the tank this morning as it progressively got worse and clearly died. The Rose is attached and has never looked much better than this picture. It is positioned towards the bottom of my tank and hasn't really moved. I run the 48'-60'' version of this light.

What are your average nitrates/phosphates/ph levels?

Sorry, all of them are in check...Nitrates and Phosphates are near zero. I run Phosban in a reactor. PH is about 8.0.

Wow certainly does not look good, what do you have for flow in the tank? How do your other inhabitants currently look and what are they? Also what wattage is that led fixture? How often do you test params?

I have two Koralia 1400/1500s, one on each side at the top of the tank. The details on the light fixture is on that link.

One other problem I'm having that may or may not have contributed to this problem...after a month in QT I added a powder blue tang to the DT about 2 weeks ago. This was about 10 days or so before I added the anemones. The fish had a terrible experience from the start--aggression from my Kole Tang, didn't eat, etc...and he died over the weekend. My other fish (2 clowns, Kole, cardinal) aren't looking healthy as a result but are doing ok. I don't think I have ich but am a bit concerned about it. Would that effect the nem?

Do you have a skimmer? I have the same light and it works great for my tank 90g.

The only thing I can think of besides swing in params or salinity that you don't know about is that the first anemone dying released toxins which really upset and may kill the nem, but that's a stretch since you took it out pretty early you said.
