Anthias with holes in head...

I got 3 carberry/threadfin anthias shipped in this past tuesday and they were all eatting and fat and healthy looking from what I could see upon arrival (they were hiding in qt)

1 day later I noticed one was breathing heavy and wouldn't eat, it was gone by that night.

One of the two that are left has what looks like 3 holes in its head above one eye that are raised, and his head looks swollen on both sides when compared to the other one (maybe one is male one female? )

First thought was maybe I got 2 males and a female and they were fighting (which I never witnessed the short time they were together) causing one death, and the other to have battle wounds?

Either way I've had kept many anthias and know they get all sorts of bacterial infections and I've had luck in the past with mardel maracyn plus antibacterial, but I just want to make sure I'm treating for the right thing.

Anyone else ever see this before? Any idea what it is?

They are in a 10g QT. one other fish that I just added yesterday BSJ. Ammonia-0

Anthias recently have been occasionally coming in with uronema marinum, so I HOPE THESE ARE IN QUARANTINE.

Yes they are still in quarantine.

I researched a bit about the symptoms of uronema marinum and most research/people who have experienced this have lost the fish pretty suddenly once the symptoms were noticeable. Both anthias have been fine now for over a week, are eatting like pigs, not scratching, not breathing heavy, not hiding or avoiding light, no white spots or velvety cottony appearance. Formalin seems to be something both this and Brooklynella are treated with, my only concern would be if it is just an open wound or lesion, would formalin or a similar chemical type treatment irritate it? Is there any harm treating a fish who doesn't have anything with formalin, especially with an open wound?
