Any idea what this disease might be?

I've been QTing a group of anthias, clownfish and a yellow tang for about 3 weeks. Tank went through a round of PraziPro and CopperSafe added to measure around 1.8 All were eating well, showing no signs of any disease.

Week 3, I started noticing some of the anthias sitting on the bottom, breathing rapidly, dead within 12 hours. A few days later, clownfish was showing signs of flashing, erratic swimming behavior and extremely rapid breathing. Also soon perished.

Yellow tang seems to be scratching, swimming against return flow and also breathing hard.

I tried to look for any signs of ICH, Velvet, Brook or Uronema but skin / fins look clean on all fish. What's baffling me is, they are exhibiting signs of ICH or Velvet but the tank had consistent level of copper in there for weeks. Any idea what this could be? Is there another disease that kills this rapidly and has similar symptoms of velvet?

I tested the copper level multiple times with API and Hach and both showed the level to be above 1.5 Is it possible that CopperSafe I used was defective? (New bottle purchased prior to QT)
 
Very rare but copper resistant velvet. How long did it take you to get to therapeutic levels? Are you also sure you were testing properly? Copper will only kill velvet when they are dinospores. If for some levels dropped, that is all it takes.
 
I spread it out over 3 days before it reached 1.8 and kept it consistent throughout. I wasn't even aware that there was copper resistant velvet. :( Any suggestion on what I can add?
