I have a beautiful fish that just can't seem to work and play well with others...at least anybody smaller than her. I'm tired of her chasing my clown and small wrasse every time they try to come out to eat. (She never tried to take on the angel.) If anyone has a tank of larger, more aggressive fish that she can't bully around and would consider trading for any interesting frag or ???, shoot me a PM.
She's a Neoglyphidodon oxyodon (common names: neon damsel; Indonesian jewel damsel; blue velvet damsel or bluestreak damsel) just under 3". The fishbase picture shows much lighter blues. Her fins are edged with more of a purple color and her horizontal stripes are deep neon blue. A more accurate picture can be found here. There's another picture on page 270 of the Scott Michaels Marine Fishes book.
These fish do lose the white vertical bar and blue coloration as they mature. Her white is still intense and extends down a little more than half of her body. Eventually, though, these fish end up at 5" and totally black in color.
(and no, I can't prove that she's female, but that's what she's always struck me as...says more about my former dating life than my aquarium keeping.)
