Bandit doing poorly. Need a diagnosis

My bandit has been with me for around 2 months and eating well. But lately I have seen this white film on its side and sometimes it twitches. Water quality is ok except that nitrates are 3.

I have dosed prazi yesterday because I suspect it's flukes, but I'm looking for your opinions.

sigh, I know, the problem is that she keeps hiding. but generally she looks like she has a layer of slime on one side and one of her fins is rotting. based on the rapid progression I believe its brooklynella. am treating with myxazin which contains both malachite green and formaldehyde, but I would be extremely sad to lose her
 
recommend kannaplex from seachem mixed with a little seachem focus and a multivitamin and a flake or pellet food to put several drops on prior to feeding the flakes or pellets.
white film = fungal infection?reason I am recommending is I had same problem /similar with my yellow tangs and I fed as above for six days and problem gone.

I see, unfortunately she stopped feeding today and looks like she might not survive the night :(

the film looks more like slime buildup though, but malachite green is also anti fungal so I'm crossing my fingers, though I can't hope for much.
 
Have you added anything to the tank that wasn't quarantined?

very stupidly yes. I thought that since I had copper in the main tank, that would be ok.

very expensive mistake.

from now on all fish have to me quarantined for a month before entering the main tank

she is dead. today is one of the saddest days I have had. a very remarkable fish with tons of personality.

I believe I found two flukes on her body. upon examination of the gills, they appeared healthy, however when I dissected her it seems that her internal organs were a mush. very unusual for a fish that has not been dead long
