Hard to really say what it could be based on a pic. You could be looking at a bacterial infection or a parasite. Either way an open wound that large needs to be treated with antibiotics. Be sure to keep the water extremely clean, vitamin enriched food and low stress level.

Kanamycin (Kanaplex) or nitrofurazone (Furan-2) are good broad-spectrum antibiotics for marine fish.
 
Update:

The poor guy died after 1 week. I was able to remove him from the tank after much aggrevation with the help of my loving wife.

I tried a a combo of lower salinity and some medicine, the spread seemed to stop and the white strings from the sore as well. Unfortunately, not sure if it was the bacterial infection (possibly) or just stress that ultimately claimed him.

I have since bought a QT again, because the last time I had a disease it was marine velvet which wiped out most of my tanks a couple of years back. I am lucky it was just a bacterial this time because nothing spread to the other fish.

I did treat the reef tank after removing him with some reef safe natural medicine just to be safe just for a couple of days in case it was viral. Corals did fine for the most part (I just have softies).
