Bicolour Anthias died suddenly?

I have (or had, rather) three of the anthias, about 2.5-3" and a Kole Tang in a 30G long QT tank. It's week three and no issues until now. Parameters are good, salinity is good, water change gets done minimum weekly, or as necessary if nitrate is elevated.

I fed them earlier today, all were eating without issue, as they do every day, then left to run some errands. I came home about three hours later to find one dead.

They are currently in therapeutic levels of copper, and have received no other medication as of yet. Prazi is intended to be dosed in a week's time.

I have observed nothing on them or in their behaviour to indicate something could have gone wrong. The only thing noticeable is that on the body, there was some slight bloat and brown discolouration on the belly. This was not present before.

I've had a fish get constipated, so it's not as silly as it sounds. Never had one die from it, though. I would lean toward bacterial infection, but that's just a guess.
 
I would be extremely surprised if that was the case considering their history. I took them off a fresh order from the box from Hawaii.

The bloat and discolouration is what's throwing me off. There is no other sign of disease on any fish.
