Black Tang help

My friend recently gave me a 15 year old black tang. The tang has been through 5-6 tank changes before going to me. I don't know what meds I need to treat his diseases. I circled the part where I think has a problem. His nose is white, upper body whiteish, his tail seems riped and he is not eating. Right now he is in qt, being treated with melafix and pimafix.

From the picture the fish is suffering from bacterial fin-rot and perhaps some fungus infection

treatment would in my opinion be with some form of antibiotics and follow that with good water quality maintenance if it survives and begins feeding again as it is an old fish, i hope it pulls though, please pay careful attention to water quality while treating it
 
looks like lateral line disease. is it eating? good water quality and vitamin soaked food and plenty of green algae[caulerpa, nori etc]. roggerreeffood.com has good vitamin soaked food and fish love it.

He hasn't been eating since I got him.
 
maracyn-2 should work melafix is garbage. use vitachem will help with the lateral line disease. use a garlic attractant or dried seaweed with garlic. soaking food or if needed live brine with vitachem and garlic. soak brine IF NEEDED for about one hour or hour and a half. also remember hes 15 so given his age it might be i little harder for him to recover. good luck!:beer:
