Blonde Naso Tank

Sorry if this is in the wrong place but wanted to get some ideas on what could be going on. I have an approximate 6" Blonde Naso Tang that I have had for about 1.5 years. He has always been a very thin fish but ate well. Within the last few days he has really started to show signs of losing weight and really isn't eating well. He picks at foods but not eating as usual. Any ideas. I have tried garlic soaking, nori, blood worms, etc. but no real difference.
 
More than likely has some type of worm. Common with Naso's in my experience. There are a number of different medicines that can be used that are effective but can't recall there names at the moment. Do a search in here as there are bound to be posts for cures.
 
Also might be flukes. If he is sratching more than normal particularly around the gills then is probably flukes and a freshwater dip is one of the best ways to get rid of them without adding any chemicals to your tank.
 
anything else inverts/livestock in the tank? what size tank?

if he goes 1 day without eating, i'd probably jump on some prazipro. you dont want to be a few days late on the treatment, then they might not eat again.
