Blue tang with abnormal black spots

Hey all! Long time lurker, first time posting. I've had this blue tang for about six months and he's developed some black spots around his eyes. Go figure, googling blue tang and black eye spots hasn't helped. Any ideas? I have a 20 gallon QT I'm dropping him in, but wondering if it's just ich or something more serious.

Other info: salinity 1.024, nitrates, nitrite, ammonia zero. Temp 79-82 throughout the day. I think I posted a pic correctly, we shall see

It being a tang having abnormal black spots I would suspect "black ich" which is not really ich but a turbellaria worm infection.

I don't have any first hand experience treating it, but google "black ich" or "turbellaria infection"and you will find more.
