Can anyone tell me if he's dying or just stressed and will return to normal or should i just cut my losses and take him out before he nukes the tank?
 
That nem is in serious trouble. It will need need immediate antibiotic treatment if it is to have any chance of making it, though he may be too far gone. How long have you had it? When did it start deteriorating? Water parameters?
 
It has nothing to do with nitrates, that anemone is sick and needs treatment with Cipro ASAP. You need to see that you find a vet TODAY to get a prescription so you can give it the first dose tonight.
You will also need a 5 or 10 gallon tank, a heater, a powerhead and some eggcrate. And you will need lots of saltwater for the daily water changes.

You know guys that not everything that gets nems sick is a bacteria infection. I think before you cipro everything you should at least check the water for NH3 and NO2.
In fact the first thing I would do before anything else is to do a 50% water change.
