Clown Fishes keep dying

Do I have to an acclimation process if i'm going to do a fresh mixed salt water ?
Do I have to keep him in QT for 2 month?
Do I have to do something with main tank (where the Brook still lives)?
 
other fish species can host brook but will not always get sick. When the clowns are re-introduced they can become ill again. The only way to eradicate the parasite in your tank is to let it lay fallow for a period of time. (I don't know the minimum time but most recommend 2 months) It is quite possible that your clowns will be fine after treatment and then going back into the tank, much like some fish will not show ich after treatment even though it is still in the tank. The concern is that if the fish gets injured or stressed in the future the parasite may take advantage of their suppressed immune system and re-infect the fish. Also, new clowns introduced are at a great risk of getting sick. This is most likely what is happening to the clownfish you've been adding.
