Clown Tang with fat lips

My tank has gone from bad to worse. Just found out I have ich in my tank the other day and now I have found that my tang may have other issues. Can anybody ID what is going on with him. It looks like he has velvet skin and his lips are really puffed up like he got punched in the mouth. Is this all it related or is this something else? Tank parameter are good. Im trying to get a picture to load but I can't seem to get it. I'll keep trying.

The stuff on the mouth is probably Columnaris, a secondary bacterial infection common with fish suffering from a parasitic infestation. When you say this fish has "velvet skin", what do you mean exactly? There is a disease called marine velvet, which can look like ich but is caused by far deadlier parasites. I think you need to get this fish in chloroquine phosphate or copper ASAP. A formalin bath might help as well, if this is velvet.
 
Ok thanks for the help. I guess I don't know how to better explain it than looking like a piece of fuzzy material all over his skin. I have a good size Sailfin that is showing no sign of anything. I have other fish but these are my only Tangs. I need to do some research on everything you said but that should get me a start. I know about copper but that would be my least favorite of the way to treat ich. I hear it's pretty hard on the fish. I realize I need to act quickly. Thanks for your help
