Clownfish help

My clownfish is getting very pale and is always hovering near sand. Whenever he tries to swim up, he flips over. I had it for around 5 months now. Used to be very active. Does anyone know what kind of parasite/bacterial disease is this?

Also, some of his skin is starting to peel off.

I think it could be brooklynella, the symptoms are: fast breathing, lethargic swimming, white stingy poop and a white thin layer of mucus covering his body.
i may suggest you to treat him with formalin baths and transfer him to a QT for a month.
Do it ASAP!!

UPDATE: He did not make it.

Now that you mentioned brooklynella, I think it might just be it. Wonder how it had it since the clownfish has been in my tank for around 5 months now.
