He has post traumatic stress disorder. Treatment should have begun immediately after the survivor had been safely removed from the crisis situation. He should receive lots of rest, a psyc eval, and surround him with the fish and critters that love him the most. Make sure he knows and understands that he is important to his new mini ecosystem and that his peers in the new tank do not want to eat him, they just want to make friends. Make sure you examine and explore his feelings of anger, shame, or guilt, which are common among survivors of trauma.
Another step in the first phase is to teach the survivor to cope with posttraumatic memories, reminders, reactions, and feelings without becoming overwhelmed or emotionally numb. Trauma memories usually do not go away entirely as a result of therapy but become manageable with the mastery of new coping skills.
I think with the right amount of love and support he will be back to grinding his teeth and digging in to a face full of sand in no time at all.
