fishes died in MP40 pump

I have two small fishes died in my MP40 pump. One fire wrasse and one clown fish. Just today, a Picasso Clown died in the pump. He was eating this morning. Not sure what happened.
Does anyone have similar experience?
 
I've never seen a healthy fish get sucked into a MP40... What parameters are you having in your tank?

Honestly... I don't run the foam guards on my 2x MP40s, and I have baby onyx clowns which are small and they hang out by them every so often...
 
I have the clown for 6 month. I have a couple anemones in the tank. But the clown never seems to host. he just stay atop of my tank.
My water parameters are pretty good. phos/nitrite/nitrate all zeros. Corals are opening up.... not sure what happened. This is the second small fishes died in MP40. All bigger fishes are fine....
 
Did you actually see these fish get sucked into the pump and die? I would tend to believe that the fish died for whatever reason, and then got sucked into the pump.
