Flame angel help

All my fish currently in QT treating ich for the last 2 months. During cupramine treatment almost lost the flame angel. Transferred to new tank and it recovered and returned with the others and moved to hypo to continue treatment. Doing fine now except has a pale face/head and twitches while swimming. Eating well and very active. Everyone else is fine. No change the past few weeks while in hypo. I'm thinking maybe HLLE? Started dosing vits with no change. Gets variety diet. ANy ideas and how to treat?
 
The twitching can mean flukes in the gills (treatable with Prazipro) OR it's possible your fish has Velvet instead of Ich. Hypo won't fully eradicate Velvet - only copper or Chloroquine will.
 
I already treated 14 days with cupramine and then have been in hypo for another 4 weeks. I have always read that velvet kills much faster than ich, within days or even hours. But can it lay dormant like ich? The flame angel is in with 3 other fish and seem normal. I will treat with prazipro next since I have not done that yet. I don’t understand the discoloration on the flame’s head though.

Full hypo (1.009) should eliminate flukes within a week. (I had forgotten about that.)

Hypo probably suppresses Velvet (i.e. kills some of the free swimmers), but won't completely eliminate it. The fish will likely still be afflicted by a sublethal concentration of trophonts.

2 weeks in copper may not have been long enough to eradicate velvet. Do you have a pic of the flame's head? It sounds like HLLE.

That's probably HLLE, but something bacterial is also possible. Might not be a bad idea to dose a neomycin based antibiotic (e.g. Seachem Neoplex) for a couple of weeks.
 
So I am slowly bringing the qt tank out of hypo and now my female clown is hanging around the surface gulping air. I just did a water change last night and no change. Everyone else seems fine. She's not doing it constantly but enough for concern. They can't still have ich! I haven't seen any other signs of it. I treated with cupramine and then weeks of hypo. Thoughts?
 
I have an air stone going and 2 simple pump filters. I did move them closer to the surface this morning to agitate the water.
