Frogspawn and other corals look dead... Idk what it is

My corals are closed up and look really bad, my frog spawn gets like 3/4 the size it use to and it has black tips on a few deals, leather coral is bent over slightly and looks bad. Idk what is going on, it look horrible, had 4 fish and now I have 2 and a shrimp. I just moved the frogspawn but here are some pictures anyways. I have a 14 gal biocube. I have done pretty regular water changes and will test all my levels today. I am replacing the filter again and doing a water change and full clean today.
 
If you want help, we need to know what your water quality is.

At a minimum we need to know:
Salinity:
Temperature:
pH:
Ammonia:
Nitrite:
Nitrate:

Also knowing the following is important, but probably not critical for your case:

Alkalinity:
Calcium:
Magnesium:

How old is the tank? What fish do you have? When was the last time you changed your lights? What size water changes do you do and how frequently?

Nitrate: under 5.0 ppm range
Nitrite: around 0 ppm
Ammonia: around 0 ppm
Ph: around 8.0, was a little under normal
Temp: 78°
Age: about 6-7 months
Lights: last changed last month

30% water change every month, with saltwater bought at the fishstore
I have an black oscellaris and another I don't remember the name of (I know that's bad) and a coral banded shrimp.
 
I'm familiar with Phishy's...been a long time customer.

You should be regularly checking your salinity as evaporation and salt creep can throw it off. I've also received incorrect salinity saltwater from high-end strictly saltwater businesses.

Also, have you noticed your clown hosting in the toadstool or frogspawn?

Okay I will check it, and no the clown hasn't. Does my frogspawn have some type of disease? The heads are turning black on a few and it's pulling from the skeleton, this may be because of the water and it could reverse so trying to do that now

Is it LED's or other. whats the rating on the bulbs.
You said the tank is 6-7 mths, but was the coral placed since then?
you changed the bulbs last mth, Was it oppened up just before you changed the bulbs or did they retract the day yo changed them..
Also as the others said, we really need to know your salinity. Forget what the LFS says. You need to check it yourself.

Well I went through the whole cycle with just sand and liverocks till all my levels were correct and then added fish, and about a few weeks after that I added one coral, then added more coral every few weeks or more. The coral did start to shrivel up before I changed the lights but I changed the lights with the exact same ones I had before.

What lights do you recommend?
 
Are you adding fresh water to compensate for evaporation? If so is it RODI or tap? Really would be benificiql to check that salinity ASAP. If I missed that in the thread I apologize.
