Frogspawn Sick?

I noticed today that a part of my frgospawn coral has separated. I attached a photo, any idea what could be going on? Any help is appreciated!

Are those flat worms on the left side of the visible skeleton?

yeah thats walling hammer,
and was it in too much flow? or something nipping at it?

Can you get it off the sand? If something is dining on it in the sand at least you can make it less easy to get to the "table"

I just caught this LPS eating worm aftger 5 years of him munching on corals in my tank.
