All bulbs should be replaced, as they have over a year of use. One of the PC's stopped firing recently, but I verified it is just the bulb. Replacement PC bulbs here:

no, it already has the ballasts. One of the PC bulbs needs to be replaced for sure, and I think the other bulbs ought to be as well.
