sure it's not brown waste? if it's receeding, it may be too late. I would tuck it under a cave away from light and lower flow and suppliment with feeding.
That's usually what they do when they're dying... they start to shred like that. If you're concerned enough to warrant drastic actions, you could try cutting the polyp off the rock and leaving the root behind. The root may grow back into a healthy shroom eventually.
