Haddoni anemone mouth extruded

I got my blue haddoni a few weeks ago. At first he settled in quite nicely, stuck on in no time but not very well. Since then he has been acting weird. First he unstuck him self and floated to another spot. I left it there a few days thinking it was not comfortable with it's original spot, but that was not the case. For a week now he hasn't stuck on to anything (rocks nor sand) and his mouth is extruded and won't go back in. Also he's only one third of the size of when i got him. I did some water tests and here are the results

Ammonia undetectable
Nitrites undetectable
Nitrate 5 mg/liter
Mg 1500ppm
Calcium 500ppm
Density 1.023
Temp 28 degrees celcius

I've had the tank for a 6 months now.

I've attached a photo. Does anyone know what's wrong? Any help would be much appreciated.

That nem is in serious trouble. You need to pull and treat ASAP. Paramus look fine except for salinity which is a bit low IMO. What do you have for lighting and flow?
 
In the photo it looks dark but for lights I have 90 leds (400 watts) plus two blue t5. (it is very bright). I have 2 3000l/h power heads. How do I treat the haddoni?
