Has anyone had this happen to their MH Bulbs?

I just got home from work and noticed this lovely suprise. One of my MH bulbs was off, I opened the canopy and noticed that one of the bulbs was black. These bulbs are only 4 months old.

Any ideas on the problem? Bad bulb, ballast???

Any help would be appreciated. I'm going to contact the manufacturer tomorrow, since my LFS where I bought it, isn't wanting to help!

bad bulb most likely. your lucky it didn't burst.
