Helfrichi Firefish had a seizure

This morning after feeding my firefish pellets like I do everyday before going to work I see him darting back and forth in the tank thumping against the glass. After 3 or 4 laps it stops shakes violently before going belly up and falling to the bottom of the tank. I've had it for over a year and I don't think I scared it as I was some distance away from the tank after feeding.

It looked like it had breathed its last breath until I was ready to get the net I came back to the tank and i was still alive. On its side barely breathing, I left it in the tank and went to work. I haven't been home yet so I don't know if its alive or not but what do you think could have caused this behavior? Did have a seizure of give its self a concussion?

Info in the setup:

Its a 20 gallon tank with a HOB filter and HOB skimmer, no heater and it lives with 2 other firefish that so no signs of stress other than when it was darting around the tank. All fish have been treated with prazi pro for 2 weeks and then hypo for a month when originally introduced.
 
The seizures are possible, yes. I used to have an ocellaris clown that wedged himself between the glass and a piece of decor, nearly killing himself in the process. Ever since that day he would have little seizures every hour or so that would last a couple seconds each time. It could be possible he choked on the pellet, my chalk bass almost died from choking on a huge chunk of nori he scarfed down. Very hard to say, but those are a few possibilities.
 
Got home and my Helfrichi looks OK. I haven't fed it yet but it looks like nothing happen to it this morning.

How could they choke on something if they don't breathe through their mouths?
 
I don't think it was choking. As this happened a few minutes after feeding.

It ate last night and looked normal but this morning it didn't eat and looks like its on its way out. Not swimming normally, looking straight up with its tail down.
