Help diagnose my xmas wrasse

I have a xmas wrasse in QT. Dosed prazi and seems no better.

Sorry the photo is sideways but you can see the large protruding stomach. He is also swimming around slowly - almost bodding through the tank, with his mouth continuously open.

I had also tried feeding peas as I read that could help alleviate the bloated stomach but he had no interest in eating that, or anything else.

Any thoughts/suggestions?

Sounds like a classic case of swim bladder (or at least an intestinal obstruction of some kind). Unfortunately, both can be hard to fix. I would keep trying the peas. The hungrier he gets; the more likely he is to eat one. I would also try some nori in a clip (all my wrasses pick at it). Nori is high in fiber and iodine; both help you go poo poo.
 
I am not about overmedicating but is there any other medicine to put into the QT aside from the prazi that may help?
Watching him it would sem he will just wilt away rather than eat. I will try to peas again and the nori, although my wrasses have never touched it.

I wish I could just get a fish enema to get the stuff out of him..........

The thing is internal worms usually create a pinched stomach; not a bloated one (at least IME). But you could try a med called "API General Cure". Look here. The two active ingredients are praziquantel and metronidazole. The latter is effective at treating internal parasites. But be warned; metro is not as "easy" on the fish as prazi.
 
He is not doing better. He is ofter stuck to the filter in the QT. i move him a bit thinking he may be gone but off he swims (not normally).

I have some inverts in the QT now. Would you do another dose of prazi? Tried the peas and he is not eating. Other suggestions? Not thinking this is going to end well but not givIng up hope.

I would go ahead and do another round of Prazi. Or API General Cure if you can QT the inverts elsewhere. It sounds like you're at the "nothing to lose by trying stage". I've been there; it sucks. :(
