HELP!!! Fish on Bottom bent

Hi,

Just came home and my dwarf flame angel is on the botom rock bent in half and cant swim?? Is there anything I can do to help him??

Gonna check the water now to see if anything has spiked
 
Ammonia 0
Nitrite is 0
Nitrate is at about 25,
I know this is high but been battling it down from 80 past 5/6 weeks

What should I do, if its a disease I want to get him out of the tank, or if I can't help him be best of putting him out of his misery.
I picked him up and have him at the top of the tank in the net just to keep other fish away from him, I put some flake food inside the net with him but he wont eat?

One thing I did notice on the last feed he wasn't very active he picked at some of the food but not what he usually would eat
 
I would put him in an acclimation box, so you can get a better look. Look for anything out of the ordinary on his skin - cut, bruise, red marks, tiny white dots, etc. Also, is he breathing heavy?

If he does have a disease, it's already in your tank (in the water).
 
he just died there, no loose rocks and didnt look like there was anything marks cuts or dots. I will be keeping a close eye on the rest of the fish.

Any idea what could have happened to him?

Is anyone able to shed some light on this, has this happened to anyone before maybe not an angel but another fish and what could have caused it?
 
minus any sort of disease, the only thing that jumps out at me would be aggression from the six-line.
