Uronema and/or a bacterial infection. I'd do the standard ridich+ bath (5ml ridich+ in a gallon of water, heavy aeration for 1 hour) and then treat in a separate system for a week with maracyn plus or kanaplex. Monitor ammonia and use ammolock or some other ammonia reducer because the antibiotic treatment is going to destroy the biofilter. I usually just do this in a 5 gallon bucket (heater, powerhead, aeration) and change the water 100% every 3 days. Transfer to a cycled QT and observe until the sore heals.
