Help Please with Flower Anemone

What would cause the changes shown in the picture below with the anemone - no changes in the environment or water chemistry? The anemone has been in the system for 6 years.

Can it recover?

Any recommendation to help it recover?

are you talking about its mouth being all puffed out?

if so how long has it been that way? and whens the last time it was feed?
 
Looks like it was agitated by something to me. I don't believe it will stay like this for long. Is there anything in the tank that could bother it?
 
To answer both - it's been this way about a week. It is not target fed but takes in when fish are fed.

Nothing near it, that little red mushroom in the picture is the only thing and it's been in the tank for 6 years.

I would start feeding it chunks of meaty food. What are you feeding the fish that it takes in?
 
I have seen them do this as a defence mechanism but have never seen one stay like this for a week. It may be starving. See if it will eat like redvipe2010 suggested.
