Help With Coral ID

Hi all
I need some help with the type of coral this is and any name it may have.
I'm not sure if it is a chalice, cyphastrea or leptoseris. Any help would be appreciated.

On my phone my first thought is Cyphastrea. If that's a standard size frag plug, those would be mighty small polyps for a Faviid.
 
I can clearly see the striations of each animal, so until a better pic or more info, still going with favia/favites.
 
it's definitely a faviid - i'm going with plesiastrea sp.

they have very, very small polyps for a faviid....often mistaken for cyphastrea
 
Thanks all for the reply's. I don't think its a favia/favites as the mouths slightly extend upwards out of the base. All the photos of favia/favites show depressed sunken mouths. This is not a standard size frag plug it is a disk about 3 inches in diameter The mouths are about the size of a pencil top eraser. The raised center section is a standard size frag plug and that is what i got it on about 1 1/2 years ago i put it on the larger disk and it has covered that and is now growing off the disk onto the rock.
 
Actually I think I may have stumbled upon the answer. I think it may be a Mycedium.
What do you guys think?

Viewing now on my computer monitor instead of phone, and hearing that's a 3" disk and not a standard plug it's on, I retract my guess of Cyphastrea.

The pigment striations surrounding each polyp looked like small tentacles on the phone, like those of Cyphastrea polyps.
