I can't figure out what's wrong!!!!!

I have a 75 gal tank with a 20 gal long sump that has been up and running since Sept. 2, 2013. I have tested my water today... Ammonia 0, Nitrite 0, Nitrate 0, Calcium 400, Alk 9, and phosphate .25(changed my GFO); but I now have lost 2 hammer corals and 1 head of a torch and I have 1 fish that is not looking too good. I don't know what is going wrong in my tank!!! I could really use some advise!!!!
 
I use the API test kits. My corals have receded. I have 48 inch dual aquatic life t5 with a 12k and a true blue actinic. My fish is not swimming as well and looks like he is having trouble breathing
 
get some better test kits, i haven't heard anything good about the api kits. with the fish haveing problems breathing i would guess ammonia is up
 
What else do you have in the tank coral and fish wise? How are they doing? What is your alkalinity and magnesium?

When was the last time you added anything to the tank?

How long was the hammer and torch in the tank before they died?
 
fish could have a bacterial infection or ich thats making it hard to breathe. does he have any wierd spots or coloration?
 
Test alkalinity, calcium, magnesium with another brand of test kit. Do a water change. Run carbon. How is your skimmer---what rating for the size of tank?
