ID and recommended treatment please

My kole tang had that when I got it. It went away by its self and he has been fine for 2 years. But I think in mine it was just where it got beat up in a net at lfs. But it looked just like that a little discoloration on its side.

However the frist kole I had died and had the same thing but he had a missive infection and died. I thin koles might just do that when stressed as mine I had for 2 years can kind of chnage to a pale color if I move rocks around and scare it then after he clams down he right back to its self.

What are you feeding it? Some times good nutrition helps more then meds

What size qt tank is it and hows the water?
 
It's a 20g long. He isn't eating because of meds. The water is pretty decent. It has some lesions and fin rot also. I think I'm going back to paraguard and using an aggressive dose regiment.
 
does it look like bleached spots? like bleach has been splattered on it? if so I had that happen to me with a purple tang that I had in qt. It was fine through a full ttm and 3 rounds of prazipro and then out of no where got these white blotches on it that looked like bleached stains. I treated with a couple of different antibiotics, formalin and paraguard and it didnt change. After doing some research I was told that it was due to stress and being in qt for too long. I decided to add it to my display and after a few days it was all gone. not sure if thats what you dealing with but might be worth looking into.
 
So, unfortunately the tang died today. I still have a starry blenny in the same QT. He has been in there since 5/3 which is approximately 60 days. The starry blenny has not shown any signs or symptoms of any diseases at anytime. He has been a healthy eater the entire time.

Do I need to continue to QT this fish? If so, for how long? All treatments have failed on the other fish should I be treating him for anything? I'm not even sure what would be the proper treatment because nothing worked and diagnosis seemed to vary slightly amongst RC members. So, moving forward...observe him for 30 days then drop him in the DT or do I even have to do that?
