I have this tailspot blenny for over two months now, but couple days ago I observed these wounds on the fish...I suppose it was due to its recent liking of this new hiding spot what should I do?
All you can really do is keep up your water quality, feed him, and keep an eye out for signs if infection. If it starts looking fuzzy or inflamed, you might have to catch him and treat him with an antibiotic in a separate hospital tank.

Keep good water quality and keep an eye on him. He might've just scratched or scraped up against a rock. With good water and being well fed, he should heal on his own.
If it gets worse though, then yes... Be prepared to set up a hospital tank.
