Is this Ich?

From the colors yours could be a Red Sea one - though it's hard to be sure with the pictures taken under actinic lights.

While the Red Sea ones have slightly more vibrant colors, the Maldives ones are much hardier and easier to keep alive.
Red Sea Pygoplites are actually known for being very difficult to keep alive, even after being acclimated. A store in Bremen, Germany had a very nice female for several years in a top notch display tank with corals. But despite that it was just dead one morning.

The slight difference in color is not enough for me to take the greater risk and paying more for it on top of that. Plus since the Maldives ones are easier to get it is also easier to put a pair together.

It is my favorite fish, right after percula clowns. As soon as I have a suitable tank I plan to get a pair again.
 
Bad news. The fish has all but stopped eating, since 3 days ago. Still active and picking at rocks. Go after the frozen Mysis but doesn't eat. One or two bites of clams then stops. The Lymphcystis symptom has gotten slightly better. But there are 4 spots on one side protruding from one side of the body. What should I do now?

Have you done the Prazi treatment yet?
She may have intestinal parasites and without getting those out you won't have much luck on fattening her back up or even to eat well.
 
I have not done that. I am thinking about a treatments in the DT. I am going to dose half doseage tonight and another half tomorrow morning if there is no issue.

I am doing the second treatment now after 5 days for the first one. The coralline seems to have taken a beating and the Acans and goniapora are not happy but I only have one of each. All the SPS are not affected. At the beginning of the first treatment the fish seems to be affected the first night but after that everyone ok and happily swimming around.

The problem is the Regal is still eating very little. One or two bites per feeding and I feed maybe 3 or 4 times per day frozen. It looks happy and even full. No sunken belly and not losing weight. The lymphcyst is gone. So what's going on here? Do they just naturally eat little or there is enough stuff on the rock to sustain it?
 
No, if they are doing fine they eat like pigs.
Maybe get her a tasty colony of zoas. Mine liked those a lot and wouldn't stop until they were all gone.
 
Both treatments are done. The Regal still take only a few bites each feeding but after 2 hrs he's ready to be fed again. What a royal pain! He is losing some weight but I can't feed every two hours. When I feed a clam he will take a bite here and there but not vigorously like other fish in the tank. I am overfeed everyone else in the tank now.

BTW very peaceful tank no bullying at all after the tangs were removed.
 
What do you feed it?
I found that they actually like algae. So maybe try some nori. And I would recommend to culture some caulerpa, but stick with the softer bubble or suction cup species.

That's one of the reasons why I don't put tangs into my tanks.
