Very difficult to see on my phone, but there's an issue there that you don't want to sit on. I'm moving this thread to the fish disease forum, you'll get quick, quality advice there.

What is your tank temp? Has it been dive bombing the sand? Is it staying out of it's hole? What type of substrate do you have? I have one and it has similar skin issues but yours looks a lot worse. I'm trying to nail it down too. I think it is an irritation to warmer water temps, I'm just guessing.

While Blue Spot Jaw fish are collected in the Sea of Cortez, and accustomed to cooler water (closer to 72F), and consequently do best with a fairly deep sand bed of fine sand which includes live rock rubble, keep in mind that many are highly susceptible to Brooklynellosis which seems to be the case here.

Thanks for the help I think snorvich is right, my sand bed is about 2" and he's always under a rock but between him and my diamond goby they move the sand around too much. What's the best route to do to cure him? And what would cause brookelynellosis? My tank temp is high 70's, don't think it was dive bombing and it was always mostly under the rock with its head sticking out until recently it's been out and that's how I notice those markings

We have a sticky in the fish Parasites sticky about brookelynellosis.
