Lazy fish: any ideas?

I've had a blue tang for a couple weeks now and everything seemed fine until the last few days. Recently he has been lethargic, swimming for a few minutes and then laying on the substrate.

When he did start to swim he would periodically scratch his right cheek on the rocks. Now it's getting worse and he rarely swims at all and he's developing ich. He still has a good appetite and will eat when I feed but he doesn't move much otherwise. I am not worried about the ich as blue tangs get this when they are stressed but I know it is a symptom of something else and I am not sure what that something is. Other than saying full spectrum anti-biotic...what treatment does he need?

So I have been watching my blue tang and trying to feed him enriched food (which is pain with my fat clowns eating everything). The tang has almost entirely stopped swimming and now has basically stopped eating. All he does is lay on his side on the bottom of the tank. I caught him and dropped him into my sump and he just lays in the chaeto like a sick guy in bed. He has dramatically lost weight and I am wondering how I should treat him? Would anti-biotics or copper be the way to go? I haven't seen a fish with these particluar issues before...increasing lethargy and now weight loss. Can anyone point me in the right direction?
