Lympho? or something else?

About 2 months ago I broke down 2 tanks due to a suspected Ich outbreak. One tank I knew was ich for sure, on my dogface puffer. The other I saw a lot of scratching on rocks but never really saw any spots, just some discolorations, a cloudy eye on a couple fish, and then a bacterial infection persued and killed my majestic angel. Since then through a ton of reading in here I think flukes was a better conclusion but I never heard of flukes back then.

All fish were pulled and both tanks have been fallow since. I treated everyone together and did TTM (7 transfers, overkill I know) until I could cycle a 90g to use for QT of these and future fish. I used maracyn plus through 2 (3 day) cycles during ttm as well.

All fish (besides a hawkfish that jumped into a bleach bucket) made it through that just fine, including a cleaner wrasse I had for about 4 months now that eats like a champ. He unfortunately liked the dogface much more than the dogface liked him, and eventually got bitten pretty good. To the point that some flesh was hanging off and he still tried to clean the dogface. He has since been moved and made a full recovery.

My question here is this. I noticed some spots on the dogface after the ttm was over. No scratching. I ran a 2 week cycle of cupramine and the spots did not go away. No new ones, just the same ones day in day out. I thought maybe flukes (even though I also used prazipro on one of the ttm cycles) so now I have prazi in the QT again for a few days but the spots are still there.

one is between his nostrils, which is several irregular shapes. One on his anal fin, which is not round at all but like a long cottony thing attached to one side. And there are several cotton like balls on his tail. These are the best looking pics I can get. It does not look like ich to me. At least not what I see on fish in the LFS's...

Also others include one flame angel, 2 maroon clowns, one fiji blue devil. All eat great, no scratching etc or any other symptoms.

Also I see many say 2 treatments of prazi. What exactly does that mean? The directions say 5-7 days. I assume run 7 days, big water change and carbon, then remove carbon and run another dose for 7 days?
 
I'm not sure what's in-between his nostrils, but what I see on the fin looks like Lympho to me.

Treat, wait 5-7 days, do a 25% WC, treat again. After that, you should be golden. It's also important to maintain elevated O2 levels during treatment.
