Mandarin - White Head??

I have a Mandarin and is head is turning white. It looks scratched up almost. He has been in the tank with no issues for about 6 months. He is still active and I believe that he is still eating. Does anyone know what this could be or how I can treat it?

The rest of the body looks fine.
 
Never seen it and I've had mandys for years. Do you have any white powder in tank? They can have things stick to their slime coat...

But we don't know the age of fish that come to us, either. It could be advancing age---just a wild guess.
 
There is a little white powder because I shifted around the rocks and cycled the sand this weekend, but unfortunately this problem was before. I feed my tank a cube of Mysis shrimp every day and about once a week i add a half a cube of cyclops. Am I feeding the mandarin sufficiently do you think?
