i was just saying that the fire fish may still be alive. I bought 2 algae blennies 2 weeks ago and ive only seen one since then, sometimes they just die for no reason
Just because the box says live fish handle with care, doesen't always mean people take heed. Fish are pretty well stressed by the time they arive at the LFS. I always try to quarintine mine for at least a week before adding them. give them a good hiding place where they can feel safe and recoup from the ordeal. I also move a few things around in the tank before I add them so the other territorial fish dont pick on them. Sadly somtimes they are sick and cannot be helped. I have a good repore with my LFS, they will replace anything that dies within 2 weeks!

IMO: Dont overstock your tank just so you can see your fish more, that is asking for a bunch of dead fish and amonia spikes up the wazoo from the bio-load that the overstock would produce.
I doubt the Kh at 13dkh killed the fish , but 13dkh is too high. Alk at a certain point would definitely kill the fish, long before it killed the fish it would burn the gills and soft tissue causing lots of problems for the fish. If you allowed it to spike by adding Ph buffer or maybe OD'd something like Purpleup then you could have easily spiked the Alk or PH and caused some problems.
What you should do is slow down and learn about proper QT and Acclimation and put those skills into practice. In the mean time get a good handle on keeping your water parameters stable.
I'm really surprised no one has mentioned this. COD could have been the damn-sels. Those fish are extremely aggressive and may have harrased the fish to death. I've seen it happen. Also, the firefish is either hiding in the rockwork or has died and been eaten by the tank's CUC. I'm guessing you checked around the floor around the tank to see if it carpet surfed while trying to get away from the damsels?
The overstocking was clearly a joke..... my LFS keeps there's at 1.220 so yeah 1.230 is cutting it, but its safe. I usually keep it at 1.240.-1.250. the fish are doing fine now. 1 christmas wrasse, 1 red madarin, and 2 perc clowns
you really need to take care when adding new fish.take it from me .i lost 2 damsels, 3clowns ,1 purple tang before i figured out it was the first 2 damsels that had ich due to careless handeling by me . now i have a fishless tank for 5 weeks to let the ich run its coarse. but three weeks i added a bunch of corals and i am enjoying them.
your tank is prolly fine. fish sometimes just die. id say buy another $10 firefish and try again.. if that little guy dies, than u proly have issues....
