Mysterious fish deaths.

I've had several mysterious fish deaths over the last few months.
The tank was setup in Oct.
I don't think there is anything wrong with my tank like copper or poisons, electrical problems.
After the tank cycled. I bought 3 firefish. One died in the acclimation bucket.
One died a week or two later. He was the smaller of the two I did see a couple of bully sessions from the bigger but not much. Then the other lasted 3 months and just disappeared.
I found a CBB that was eating and he was eating for a couple few weeks then stopped eating and died.
I bought 4 lyretail anthias. One little female I thought was dead reappeared weeks later. The male never ate and died within a week. One big female was very aggressive and eating right away. She is still with me along with a timid female. One female died within a day.
So I didn't add any fish for a while and last week i got a Melanuras wrasse.
It didn't eat flake/pellet for a couple days then about day 3 it ate some mysis and bloodworms. It died today. It seemed healthy and was eating pods off the rocks all day since I put it in the tank. It only lasted 4-5 days and was about 4" and fat.
I do not have any bullies in my tank. I have peaceful Large Naso, Small Hippo. The two Anthias are friendly now. And a yellow watchman. 215 gallon.
I understand the Copperband is not easy but am really perplexed with the Melanuras and firefish. Inverts seem happy. Water is excellent normal parmas. SPS are happy and growing.
I do regular water changes, alk, SG are always stable about 1.024-6. PO3 .03-.05 , NO3 0 on API kit.
I know it's nearly impossible to answer these type of threads but was wondering am I the only one with many mystery deaths?
 
Most likely disease related, did the dead fish have any signs and symptoms on their body? You didn't mention QT so I assume you don't practice QT?
 
No signs of illness. I don't have a QT tank.
Fish seem fine in the store. And some fish seem fine in my tank for a while. The two lyretails that died I could tell something was wrong because they would not eat. The firefish did eat but one was small and one big so maybe the big little one was stressed.
Forgot I also have two clowns that seem happy.

+1 There are several diseases that established fish are more resistant to. Ich, gill flukes and intestinal parasites all come to mind. Ich can harbor in the gills, and with flukes/intestinal parasites - all you may see is white stringy poop. And who really pays attention to the color of their fishes poop?!
