Mystery Wrasse in trouble

i have had a mystery wrasse for about 4 months and he has been fine. i came home from school and could not catch him so i just had my roommate feeding him. i went back up and could not find him so i removed all my rock and finally caught him. he was not looking good but not sure why. he has very dark patches on him and he is hidding in the corner being very lathargic. he is also not eating well. i am feeding him flake w/garlic and mysis. i also think he had ich but that has cleared up. i have no pics so i know it makes it a lot harder but any suggestions would help. he is in QT right now. he was getting pick on by a pseudochromis for a few hours but that was not what caused it just didnt help.
