I have had this Picasso true perc. for about 12 months maybe a little longer. He is have skin issue recently and stopped hosting. I'm not sure it's related

He was hosting a LTA. The skin irritation has got worse over 3 month period. He's had it for about 4 months total. No new arrivals at that time. No other fish show signs.

Is it still eating? I find that parasites like brooklynella and velvet progress quickly. The fish stops eating, goes from being lethargic to half swim/ half float until it dies...typically within 14 days or less even with medication present. So based on that I think you are dealing with a bacterial infection and appropriate treatment should start as soon as practical. I like using Maracyn 2 but Furan 2 should also work. You should also try quick-ich X bath for 45-60 mins even if it's just in a 1 gallon container while at the same time treating for infection in a separate quarantine tank. That way you will avoid mixing medications and causing potentially adverse effects.

Normally yes, but in the stores around here I've seen clown fish with clear brook infection staying alive for a month or more as long as they are otherwise strong. Uronema can even take longer to take an otherwise strong fish down. Mostly new fish die quickly because they are weakened from all the stress they have been put through.

The more I look at that picture the more it looks like it could be Uronema.

I would for sure put at least one formalin dip on the treatment plan and then treat with antibiotics in the hospital tank.
