Need fast help with yellow tang

So my yellow tang is showing symptoms such as rapid breathing and scratching and some sporadic swimming. I don't believe it to be ich or flukes as its in QT and I've been treating for both with cupramine and prazi pro. Mind you, this is the second yellow tang that has showed the same symptoms in quarantine (the first one died after a failed freshwater dip so not doing that again)

Is there any other disease out there that has these same symptoms that isn't flukes or ich or velvet? I'm at a little bit of a loss.

No noticeable spots. Water params are good. It's a constantly setup qt that has been setup and fully cycles for quite a while now so I don't suspect that's the issue. Is it possible low oxygen levels could cause something like this?
I just dropped in an air pump on the chance that could be it.
 
a QT cant be constantly cycled if fish are coming in and out. The bacteria dies off and Im guessing your seeing spikes in ammonia or nitrite. pull out the kit and check again.

the other possibility is bacteria infection from bad water quality if the QT is not going through routine water changes

My guess would be velvet - copper is hit or miss with it.
I would do a formalin bath and then administer CP in a sterilized HT.
And do not add any bacteria starter products, rather feed sparingly.
I would also drop the salinity to 1.018
