need help with my acro

I have had this guy for about a year, he's my first and only sps other then the one behind him. I don't really know what type he is but I love him. However I had to move my tank two weekends ago and he seems to be dieing off in the center. Please help!

Tank specs:

100 g display
50 gallon sump fuge

Tank params a bit off due to the move I'm doing 10 gallon water changes about once every two days now to get them back to normal. I'll post them once I have a complete list.

T-5 lighting 6 bulbs everything growing great!

Frag a couple branches and glue to some other rock. If the colony continues to die maybe you can save the strain.

This morning the entire coral was pretty much gone. I found some super glue and had at it with what was left, we'll see.....
