Never seen anything like this before, It was munching on an SPS frag. 5 inches long and all soft tissue:

I've been reefing for 15 years and I have never seen anything like this. That is truly creepy.

Same thing we said, we are a combined 30 years in the hobby and have no idea either. Some have said coco worm or feather duster but we have seen them out of their tubes before. Showed it to our friend at an LFS and he also confirmed not a coco worm/feather duster. The leading candidate was sea pen but again no idea.
 
Normally I would not disagree with you, but our mimic blenny killed our last coco worm 8 + months ago... Before we pulled and etched the live rock in our tank. Which all went back in before Thanksgiving after being etched and cycled again. Believe me there were no coco worms left in the tank. Nor were any of the ones we had of that size. All would have been (if the blenny had not killed them) less than 3".
 
Well then maybe it was another similar worm living somewhere else where you couldn't see it? I mean I'm positive that's what this is. I'll try to drag LeslieH in here though if you want ;).
