now even more confused... red carpet

I am sorry for all the posts, I am just really concerned about my carpet nem and want to be able to do whatever I can to make sure he stays healthy/ if I need to do something to help him.

He ate a piece of tilapia today, but his "bubble" is still out.... I am starting to think it is something like an "air bladder" fish issue... I am getting more and more tempted to put a thin hollow needle through it (long enough to reach the air and him) and let the air out because I am not sure this bubble is affecting his health or not.

If you look at the yellow circle, inside you can see the piece of tilapia and then, after he eats it, it is gone - yet he is doing this with his "bubble".

Has anyone ever seen something like this before or have any experience or suggestions for it?

If I am you, I would not feed him Tilapia because this is a fresh water fish.
I would concentrate on provide good condition for him. Check your water and make water condition optimal.
That bubble is a liquid bubble, not an air bubble. Do not stick a needle into it. This will only stress him more.
 
I have had Maxi Mini with similar condition, It resolved after a few weeks after I pay more attention to condition of the water of my tank.
 
My red haddoni has had the same bouble as your but much smaller a year ago. I did not do anything but tried to improve the living environment for it (light, water quality...).
On another note, I almost never feed my anemones, they all are still very big and healthy. Hope it help.
 
As suggested above don't poke it with a needle and try feeding smaller food like mysis shrimp...might be a good idea to stop feeding it for a week or two to see if that helps. If the nem is stressed and needs time to recover, feeding it might not be helpful.
