You folks were looking for Ctenochaetus flavicauda
I think this one is Ctenochaetus binotatus
both have color variations within the species.....
FWIW my "holy grail" of Ctenochaetus is the blue striped bristletooth which can be seen on page 76 of the Kuiter/Debelius book "Surgeonfishes Rabbitfishes and their Relatives"
