Powder Brown Stung?

I came home today and my Powder Brown has a mark on the top of its head that looks like it may have bumped in to my Long Spined Urchin. He is swimming and eating but I may be seeing early signs of ich from the stress. My question is not about ick (can anyone say beat a dead equine with a stick) but has anyone else had this happen. This bump/pock mark was not present this morning and was full on when I got home so I think it was more trama than a developing problem and the Urchin is the only thing I can come up with. Thanks in advance for any info.
 
I inspected the affected area more closely and the center is dark (similar to the fishes body color) and the area around it is pale and raised like it swelled and lost color. The total diameter of the area is about a quarter inch.
 
I think my PBT is the klutz of the tank. He is constantly running into things, scraping himself up, and occasionally taking a stab from a fellow tang. To date, I have not had to pull him out and treat a wound. They generally heal in 10-14 days uneventfully. Good water quality is key!
 
I do have a Yellow Tang as well and I thought at first that he might be the culprit because he does try to back his barb in to the PBT on occasion but the location of the wound makes me wonder if that is possible. I think the wound will heal but I am more concerned about keeping the stress down so ick doesn't become a problem. Thanks for the reply.
