RBTA Problem

Hi, hope someone can help me, I have a RBTA which has been in the tank for two months under max spectrum led lights. The last two weeks its loosing its tentacles, can someone tell me what might be the issue. I have attached a picture. Thanks

Alk is at 13, Po is at .25 ppm, Sal is at 35ppm, Cal is at 460ppm, the only thing high is nitrates at 30ppm.
 
Seems like if it were a water issue the whole anemone would be having problems, not just that side and only in the tentacles. I would check it with a red light at night just to rule out the possibility of a small creature eating it. Otherwise, did maybe a rock fall onto that side that may have pinched the tentacles and caused them to slowly fall off? Its just really weird that only one side of the tentacles would just randomly fall off and leave the rest looking fine.
 
Not true... ur alk is lityle to high. I had same pblem with mine it started making funny shapes and eatting it tenticles on opposite sides of the nem. Is it turning a grey color? And if ur tank is not stable then nem will not be happy
 
I've seen them look like that in the store. I assumed they were not being fed. Has it been eating at all?
 
Same question as the others … is it eating? If it is then my guess would be a Gorrilla Crab or some other predator.
